Return-to-Campus COVID-19 Testing: Monday, January 3, 2022

STUDENTS
High Point Academy will provide a drive thru testing clinic for all students on Monday, January 3rd from 8:30am-1:00pm. For drive-thru testing, please line up in carline at the front of campus (Kinneloa Canyon Road & Eaton Canyon Road). Please plan to have your student tested in the time slot designated for them by their last name. Please note that any student diagnosed with COVID-19 in the past 90 days should not participate in this testing even t unless they are currently having COVID-19 symptoms.
Please make sure you have the following required forms printed and with you for testing:
- On the day of testing, bring this completed testing requisition form: Click here to download the PMH Requisition Test Form. Complete only the highlighted sections.
- For the test type, please check the box for “RT-PCR Swab,” then mark “Oropharyngeal Swab” (which is a cheek swab) if your student is under the age of 12. Please mark “Anterior Nares Swab” if your student is 12 years or older.
- On the day of testing bring a printed photocopy of your child’s health insurance card. If your child does not have health insurance, please complete this form: Click here to download the Cares Act Uninsured Form.
Students under the age of 12 will receive a cheek swab for their test. If your student is under the age of 12, please ensure that the student does not eat or drink 30 minutes prior to testing, as this may affect results.
Testing times on January 3rd are arranged by student last name. In order to maintain the best traffic flow, please make every effort to arrive during your designated time slot. If that is a hardship, come when you can:
8:30am-9:15am: Student last names from Abadjian to Dishchekenian
9:15am-10:00am: Student last names from Dohlen to Keller
10:00am-11:00am: Student last names from Kennedy to Mispagel
11:00am-12:00pm: Student last names from Mohta to Taft
12:00pm-1:00pm Student last names from Tam to Zou
